It lies entirely within South African territory, with not a metre of border with any third country. Only three states are like this: San Marino and the Vatican, both inside Italy and both very small, and Lesotho, roughly the size of Belgium with more than two million people.
Because everything entering or leaving Lesotho passes through South Africa — every international flight, container, power line and export shipment. The practical value of a Lesotho passport depends heavily on relations with a single country. That is the highest concentration risk a nationality can carry, and it appears in no passport ranking.
Don't only count visa-free countries. Ask whether your mobility is concentrated in one relationship and how stable it is. A passport reaching fewer countries but resting on many independent treaties can be more durable than a high-scoring one resting on a single tie.
All of Lesotho's territory lies above 1,000 metres — the only country in the world of which that is true. Rain falling in Lesotho runs down into South Africa, and Lesotho has built major dams to sell water to the industrial Gauteng region containing Johannesburg and Pretoria. It is one of very few cases where fresh water is a formal export under treaty with steady budget revenue.
Because the terrain makes each side's position so clear there is nothing to argue about. Ethiopia disputes Nile water use, Malawi disputes a lake surface, the Central Asian states dispute flows — Lesotho is the only case where the two sides buy and sell. Water disputes arise where rights are unclear; where rights are clear, it becomes a market.
Water sold to South Africa, garments exported to the United States under trade preferences, diamonds, livestock and wool, and remittances from Basotho working in South Africa. The garment sector is highly sensitive to whether those trade preferences persist.
Both countries sent a generation to South African mines, but Basotho could return because Lesotho is right inside South Africa with labour agreements in place. That is why Lesotho did not produce a large-scale statelessness episode of the Malawi–Zimbabwe kind.
